Important: Please Read First
Some of these forms are available in a "Fill-in" format, meaning you can view, complete, and print the form on your computer. Currently there is no computation, validation, or verification of the information you enter, and you are still responsible for entering all required information. Instructions may require some information to be handwritten on the form (e.g., signatures).
NOTE:The free Acrobat Reader does not allow you to save your completed forms. You should print two copies to have one to keep for your records. If one attempts to "SAVE" the form, the DATA in the form will be DELETED permanently by the Acrobat Reader program.
If you experience problems with downloading forms, it is recommended that you upgrade to Adobe Reader 6.0 or higher. Please note that the"Fill-in"forms are a larger file size and will take longer to download.

(Nebraska Revised Code of Judicial Conduct, Appendix C)All Nebraska judges, clerk magistrates, child support referees, and retired judges consenting to temporary judicial service are required to file this statement by May 1 of each year with the Clerk of the Supreme Court.

Forms to be filed with the Administrative Office by judges with cases over 90 days. Separate forms for County/Juvenile, District, and Workers’ Compensation Judges.
